QUINCY: David Forman, 48, of Silver Street died April 23, in his sleep at home.
He leaves his wife of 18 months, Jacqueline Placide, a son Casey Shane Forman of Stoughton, Casey's mother, Livia Soares of Stoughton, parents Jeffrey and Barbara (Arel) Forman of Eastford, CT, a step daughter Kristen Dirksmeier of Quincy, many aunts, uncles and cousins.
He was born in Boston, graduated from Stoughton High School and worked towards his degrees in criminal justice and culinary arts from Massasoit College.
David was a chef for Milton Hospital for many years and lived in Stoughton for most of his life.
He enjoyed cooking, spending time with his son exploring amusement parks, fishing and video games. He also enjoyed watching the series Walking Dead and thriller movies. David was also an avid sports fan.
Calling hours are 2 to 4PM, Sunday, May 1, in the Belanger-Bullard Funeral Home, 51 Marcy St. Southbridge.
A service will take place at 4Pm in the funeral home.
